Background Check Disclosure & Authorization

This document is provided to you in compliance with the federal Fair Credit Reporting Act (FCRA). It is intentionally standalone — it does not contain any other agreement, liability release, or terms of service. Please read it carefully before authorizing a background check.
1. Disclosure regarding background investigation
In connection with your application for engagement as an independent contractor with Unify, Orqestra LLC (“Unify”) may obtain a consumer report about you for employment-related purposes.
The consumer report may include information regarding your credit history, criminal background, driving record, sex offender registry, character, general reputation, personal characteristics, mode of living, and information from other sources.
The report will be prepared by Checkr, Inc., a consumer reporting agency located at One Montgomery Street, Suite 2400, San Francisco, CA 94104. Checkr's privacy policy is available at https://checkr.com/customer-privacy-policy. You may contact Checkr at support@checkr.com or by phone at 844-824-3257.
2. Your rights under the FCRA
You have rights under the Fair Credit Reporting Act, including the right to know what is in your file, the right to dispute incomplete or inaccurate information, and the right to a free copy of your report under certain circumstances. A full summary of your rights is available here:

Please review that document before authorizing the background check. You may also obtain a printed copy from us on request at no charge.
3. State-specific notices
Unify currently operates only in the State of New Mexico. New Mexico does not require additional state-specific consumer-report disclosures at this time. As Unify expands into other states (for example, California, Massachusetts, New Jersey, New York, or Washington), additional state-specific notices required by those jurisdictions will be appended to this document and presented to you before any report is procured under that state's law.
4. Authorization
By providing my consent in the Unify app or website, I authorize Unify and Checkr to procure a consumer report about me for employment purposes. I understand this authorization remains valid for the duration of my Unify account unless I revoke it in writing.
I acknowledge that I have received and read the disclosure in Section 1 above, and that I have been provided a link to the Summary of Rights described in Section 2.
5. If a report affects a decision about you
If Unify intends to take any adverse action (for example, denying or terminating your engagement) based in whole or in part on information contained in a consumer report obtained about you, the FCRA requires us to follow a two-step process:
- Pre-adverse action notice: Before taking action, we will send you a copy of the consumer report and a copy of the Summary of Your Rights Under the FCRA, and we will allow a reasonable period (typically at least five business days) for you to dispute the accuracy or completeness of the report directly with Checkr.
- Adverse action notice: If, after that period, we proceed with the adverse decision, we will send you a final adverse-action notice that identifies Checkr as the consumer reporting agency, states that Checkr did not make the decision and cannot explain it, and informs you of your right to obtain a free additional copy of the report from Checkr within 60 days and to dispute its accuracy.
6. Revoking authorization or requesting a copy
You may revoke this authorization, or request a copy of any consumer report Unify has obtained about you, by contacting us at:

Revocation will not affect any action taken in reliance on this authorization before we received your revocation, but will prevent any further consumer reports from being procured. We will respond to revocation and copy requests within 30 days of receipt.
